OFFICIAL SWEEPSTAKES RULES
VOID WHERE PROHIBITED. NO PURCHASE, PAYMENT, OR CONTRIBUTION
NECESSARY TO ENTER. A PURCHASE, CONTRIBUTION, DONATION, OR PAYMENT
OF ANY KIND WILL NOT INCREASE YOUR CHANCES OF WINNING. BY ENTERING
THE SWEEPSTAKES, ENTRANT AGREES TO BE BOUND BY THESE OFFICIAL RULES.
1. Sweepstakes Overview: Breakthrough T1D, a tax-exempt nonprofit organization described in Section
501(c)(3) of the Internal Revenue Code, is conducting a Sweepstakes (the “Sweepstakes”) to solicit
contributions. By obtaining an Entry in the Sweepstakes, the entrant (“Entrant” or “You”) agrees to be
bound by these Official Rules (“Rules”). The Sponsor’s interpretation and application of the Official
Rules shall be final.
2. Eligibility: The Sweepstakes is open to individuals who are legal residents of the 50 United States and
the District of Columbia and are 18 years of age or older. Employees of Breakthrough T1D (the
“Sponsor”), their respective affiliates, subsidiaries, and their immediate family members and/or those
living in the same household of each are not eligible to participate in the Sweepstakes. The
Sweepstakes is subject to all applicable federal, state, and local laws and regulations. The Sweepstakes
is void where prohibited by law.
3. Agreement to Rules: By participating, You agree to be fully unconditionally bound by these Rules, and
You represent and warrant that you meet the eligibility requirements set forth herein. In addition, You
agree to accept the decisions of Sponsor as final and binding as it relates to the content.
4. Sweepstakes Period: In-person entries will be accepted at the Tampa Gala starting at 6:00 PM EST on
November 22, 2025 and ending at approximately 9:00 PM EST on November 22, 2025. Winner will be
selected as described herein in Section 8 below at approximately 9:00 PM EST on November 22, 2025.
The Sponsor’s database computer is the official time-keeping device for the Sweepstakes.
5. How to Enter: Participants may enter either by the Contribution Method or by the Free Entry Method,
as described herein:
(a) Contribution Method: To enter by contributing money to Sponsor, you must make a contribution
of $100 to Sponsor and follow the instructions to enter, which will include providing your first name,
last name, phone number and email address by November 22, 2025 at 9:00 PM EST. Fund A Cure
gifts do not qualify as sweepstakes entries.
(b) Free Entry Method: To enter via the Free Entry Method, you must submit an entry by following
the instructions to enter, which will include providing your first name, last name, phone number and
email address by November 22, 2025 at 9:00 PM EST.
For all entry methods:
There is no limit as to the number of times you may enter. No mechanical reproductions will be
accepted. The entry must fulfill all Sweepstakes requirements as specified herein to be eligible to win
a prize. Entries that are incomplete, illegible, or do not adhere to these Official Rules or specifications
may be disqualified at the sole discretion of Sponsor. You must accurately provide the information
requested. You may not use multiple email addresses, identities, or devices to circumvent the rules.
Proof of contribution entry or free entry will not be deemed to be proof of receipt of entry by Sponsor.
If you use fraudulent methods or otherwise attempt to circumvent the rules, your submission may be
removed from eligibility at the sole discretion of Sponsor.
6. Prizes: There will be one prize winner, and there will be one prize. Each entry gives you one (1) chance
to win a prize. Each Prize and the Approximate Retail Value (“ARV”) of each prize is as follows:
Package including Tory Burch Spaghetti Strap Leather Bucket Bag and bottle of Weller Special Reserve;
ARV $975
Any difference between the actual retail value of a prize and the ARV will not be awarded. The actual
retail value may vary depending on fluctuations and other variables. No substitution, assignment, or
transfer of any prizes are permitted, except by Sponsor, who reserves the right to substitute the prizes
with another of comparable or greater value.
The potential winner must sign and return to the Sponsor, within five (5) days of the date of notice or
attempted notice is sent, an Affidavit of Eligibility and Liability Release to claim the prize. Sponsor may
also ask for a signed Affidavit of Publicity Release.
Note: The Affidavit may require the Winner to provide their Social Security Number to the Sponsor,
which will be used solely for tax reporting purposes. Actual or appraised value may differ at time of prize
award. The specifics of the prize shall be solely determined by Sponsor. No cash or other prize
substitution shall be permitted except at Sponsor’s discretion. All prize-related expenses, including and
without limitation, all federal, state, and/or local taxes, shall be the sole responsibility of the winner.
Acceptance of the prize(s) constitutes permission for Sponsor to use Winner’s name, likeness, and entry
for purposes of advertising and trade without further compensation, unless prohibited by law.
7. Odds: The odds of winning depend on the number of eligible entries received from all methods of
entry. There will be 100 entries available for the sweepstakes in total.
8. Winner selection and notification: The winner of the Sweepstakes (“Winner”) will be selected in a
random drawing by a computer under the supervision of the Sponsor on November 22, 2025. The
Winner should be present at the event to win. If the selected Winner declines the prize, cannot be
contacted, is ineligible, fails to claim the prize within 0 after the random drawing, or fails to timely
return a completed and executed declaration and releases as required, the prize may be forfeited
and an alternate Winner selected.
The receipt by Winner of the prize offered in this Sweepstakes is conditioned upon compliance with all
federal and state laws and regulations. ANY VIOLATION OF THESE OFFICIAL RULES BY ANY WINNER (AT
SPONSOR’S SOLE DISCRETION) WILL RESULT IN SUCH WINNER’S DISQUALIFICATION AS WINNER OF THE
SWEEPSTAKES AND ALL PRIVILEGES AS WINNER WILL BE IMMEDIATELY TERMINATED.
9. Taxes: Winner must note that any prize with ARV or fair market value equal to or greater than $600
is taxable income to Winner, and Sponsor will report the actual value of the prize to the Internal
Revenue Service via Form 1099-MISC. Winner agrees to provide their valid name, address, and Social
Security number to Sponsor for tax reporting purposes, if required. The value of the prize is
includible in Winner’s income for the year in which the prize was transferred to Winner. Winner is
solely responsible for all matters relating to the prize after it is awarded. WINNER IS RESPONSIBLE
FOR PAYING ALL FEDERAL, STATE, AND LOCAL INCOME TAXES, AND OTHER APPLICABLE TAXES THAT
ARE DUE IN CONNECTION WITH RECEIPT OF THE PRIZE.
10. Rights Granted by You: By entering this contest, You understand that Sponsor, anyone acting on
behalf of Sponsor, or its respective licensees, successors and assigns will have the right, where
permitted by law, without any further notice, review or consent to print, publish, broadcast,
distribute, and use, worldwide in any media now known or hereafter in perpetuity and throughout
the World, your entry, including, without limitation, the entry and Winner’s name, portrait, picture,
voice, likeness, image or statements about the Sweepstakes, and biographical information as news,
publicity or information and for trade, advertising, public relations and promotional purposes
without any further compensation.
11. Terms: Sponsor reserves the right, in its sole discretion, to cancel, terminate, modify, or suspend the
Sweepstakes should (in its sole discretion) non-authorized human intervention, fraud, or other
causes beyond its control corrupt or affect the administration, security, fairness, or proper conduct of
the Sweepstakes.
12. Limitation of Liability: By entering, You agree to release and hold harmless Sponsor and its
subsidiaries, affiliates, advertising and promotion agencies, partners, representatives, agents,
successors, assigns, employees, officers and directors (“Released Parties”) from any liability, illness,
injury, death, loss, litigation, claim or damage that may occur, directly or indirectly, whether caused
by negligence or not, from (i) such entrant’s participation in the sweepstakes and/or his/her
acceptance, possession, use, or misuse of any prize or any portion thereof, (ii) unauthorized human
intervention in any part of the entry process or the Promotion; (iii) electronic or human error which
may occur in the administration of the Promotion or the processing of entries. Winner agrees that
the Released Parties shall not be liable for any loss or injury resulting from participation in the
Sweepstakes, or the acceptance, use of any prize, or participation in any event associated with or
comprising a part of the prize, including any travel to any destination related to the prize, including
to and from the airport, or to and from any event related to any prizes, and any and all events and
travel related to the prizes.
13. Disputes: THIS SWEEPSTAKES IS GOVERNED BY THE LAWS OF THE UNITED STATES OF AMERICA AND
THE STATE OF NEW YORK, WITHOUT RESPECT TO CONFLICT OF LAW DOCTRINES. As a condition of
participating in this Sweepstakes, Entrant agrees that all disputes which cannot be resolved between
the parties and causes of action arising out of or connected with this Sweepstakes, shall be resolved
individually, without resort to any form of class action, exclusively before a court located in New York
having jurisdiction. Further, in any such dispute, under no circumstances will Entrant be permitted to
obtain awards for, and hereby waives all rights to claim punitive, incidental, or consequential
damages, including reasonable attorneys’ fees, other than Entrant’s actual out-of-pocket expenses
(i.e., costs associated with entering this Sweepstakes), and Entrant further waives all rights to have
damages multiplied or increased.
14. Privacy and Privacy Policy: You consent that any personal information (i.e. contact information,
payment information, address, phone number, email address, etc.) provided may be collected and
used in connection with administering the Sweepstakes, including, but not limited to, contacting and
announcing any winners. Additionally, information provided by You will be used and governed in

15. Winners List: To obtain a copy of the winners list or a copy of these Official Rules, mail such a request
along with a stamped, self-addressed envelope to: Breakthrough T1D.
16. Sponsor: The Sponsor of the Sweepstakes is Breakthrough T1D, 200 Vesey St, 28th Floor, New York,

17. Bound by These Rules: You, as Entrant, agree that, by submitting an entry, you will be bound by
these Official Rules and you have affirmatively reviewed, accepted, and agreed to all of the Official
Rules and other conditions.
